Following promulgation of the Nuremberg code in 1947, the ethics of research on human subjects has been a challenging and often contentious topic of debate. Escalation in the use of research participants in low-income countries over recent decades , has intensified the debate on the ethics of international research and led to increasing attention both to exploitation of vulnerable subjects and to considerations of how the 10:90 gap in health and medical research could be narrowed. In 2000, prompted by the (...) discussions over several years that led to the US NIH launching a capacity building programme on research ethics for members of research ethics committees in developing countries, we advanced a ‘new look’ for the ethics of international research.1 Since then progress has been made on several fronts.First, our ideas—considered somewhat radical and impractical at the time—have been provocatively addressed by scholars who have either contested them or advanced similar conceptions of what obligations international researchers have to research participants and communities in low income countries before, during and after clinical trials. Second, those researchers who have been sympathetic to our ideas have either endeavoured to put these into practice or have investigated the feasibility of doing so. Third, the intractability of the 10/90 gap and the escalation of interest in global health have sensitised many to the need to amplify the uptake of these ideas in practice.Here, we briefly review the conceptual and practical developments in international research ethics. Originally conceived as a forty-page conclusion to Hacker’s twenty years of work on the monumental four-volume Analytical Commentary on the Philosophical Investigations, this book “rapidly assumed a life of its own”. A major contribution to the history of analytic philosophy, this substantial volume delivers even more than the title promises. The eight chapters are best approached as a six-chapter book, itself some 220 pages long, on Wittgenstein’s contribution to twentieth-century philosophy, followed by a two-chapter, 120-page epilogue about how and why (...) his influence has waned. The first six chapters provide an encyclopedic summary of the fruits of Hacker’s research on Wittgenstein’s writing, an immensely learned account of British philosophy from the turn of the century to the 1970s, and a detailed account of Wittgenstein’s reception by Oxford, Cambridge, and the Vienna Circle. The book’s closing chapters, “Post-positivism in the United States and Quine’s Apostasy” and “The Decline of Analytic Philosophy,” polemically argue that Quine’s philosophy, and the post-Quinean naturalism prevalent in Anglo-American philosophy today, amount to such a decisive break with the analytic tradition, as Hacker conceives of it, that they should not be counted as “analytic.”. In his discussion of Roman military institutions Polybius described how the desire for fame might inspire Roman soldiers to heroic feats of bravery, including single combat: τ δ μέγιστον, ο νέοι παρορμνται πρς τ πν πομένειν πρ τν κοινν πραγμάτων χάριν το τυχεν τς συνακολουθούσης τος γαθος τν νδρν εκλείας. πίστιν δ' χει τ λεγόμενον κ τούτων. πολλο μν γρ μονο-μάχησαν κουσίως ωμαίων πρ τς τν λων κρίσεως κτλ. Modern scholars, however, have taken little notice of this remark and some (...) have tried to belittle the importance of single combat at Rome. Thus G. Dumézil alleged that the Romans fought few single combats and that this was significant for their outlook upon war, while R. Bloch described the duels in the seventh book of Livy as ‘un mode de combat absolument étranger à la tradition romaine, mail auquel les Romains ont été contraints par les habitudes et par les défis des Celtes’. W. V. Harris is the only scholar to have understood the importance of monomachy in the Roman Republic, but even he has not assembled all the evidence necessary for an accurate assessment of the phenomenon. Antiphon's speech on the murder of Herodes has been variously dated by several scholars, but all seem to agree that it was delivered a good many years after the revolt and recapture of Mytilene. According to this opinion the speaker in § 74 declares himself too young to know much of what happened in those days. Before going into this more carefully, it seems necessary to visualize the situation of the accused man. In order to achieve this the best we (...) can do is to follow his own words in the Sirjyrjcris, §§ 20–24: ‘I travelled from Mytilene on the same ship as the lost man; we were on our journey to Ainos, I to see my father—he happened to be there at that time—he in order to sell some slaves to Thracian natives. These natives, who were to pay the ransom, and the slaves were on board too.—All this I will prove by witnesses.—As we, however, met with stormy weather, we were compelled to put in to a harbour in the territory of Methymna; there we found that* ship into which Herodes changed; in that ship, according to the accusers, he died. Now all this was mere chance: obviously I had not persuaded him to travel with me, no, he was travelling on his own business. Then I, personally, had a very good reason for travelling to Ainos; our putting in to that harbour was not premeditatd, we were compelled to. And again, when we were lying at anchor, the change of ship took place without any trickery or fraud: again, it was done because of necessity. The ship, viz., on which we had been travelling was not covered, the other one on the contrary had a covering; that was because of the rain.—All this I will prove by witnesses.—After boarding the other ship we drank some wine. In recent years philosophers have given much attention to the ‘ontological problem’ of events. Donald Davidson puts the matter thus: ‘the assumption, ontological and metaphysical, that there are events is one without which we cannot make sense of much of our common talk; or so, at any rate, I have been arguing. I do not know of any better, or further, way of showing what there is’. It might be thought bizarre to assign to philosophers the task of ‘showing what (...) there is’. They have not distinguished themselves by the discovery of new elements, new species or new continents, nor even of new categories, although there has often been more dreamt of in their philosophies than can be found in heaven or earth. It might appear even stranger to think that one can show what there actually is by arguing that the existence of something needs to be assumed in order for certain sentences to make sense. More than anything, the sober reader will doubtlessly be amazed that we need to assume, after lengthy argument, ‘that there are events’. (shrink)

The rule of rescue describes the powerful human proclivity to rescue identified endangered lives, regardless of cost or risk. Deciding whether or not to perform a decompressive craniectomy as a life-saving or ‘rescue’ procedure for a young person with a severe traumatic brain injury provides a good example of the ethical tensions that occur in these situations. Unfortunately, there comes a point when the primary brain injury is so severe that if the patient survives they are likely to remain severely (...) disabled and fully dependent. The health resource implications of this outcome are significant. By using a web-based outcome prediction model this study compares the long-term outcome and designation of two groups of patients. One group had a very severe injury as adjudged by the model and the other group a less severe injury. At 18 month follow-up there were significant differences in outcome and healthcare requirements. This raises important ethical issues when considering life-saving but non-restorative surgical intervention. The discussion about realistic outcome cannot be dichotomised into simply life or death so that the outcome for the patient must enter the equation. As in other ‘rescue situations’, the utility of the procedure cannot be rationalised on a mere cost–benefit analysis. A compromise has to be reached to determine at what point either the likely outcome would be unacceptable to the person on whom the procedure is being performed or the social utility gained from the rule of rescue intervention fails to justify the utilitarian value and justice of equitable resource allocation.
